How to
Use the Surveys / Outcome Measurement Tool

The Surveys / Outcome Measurement Tool can track members’ school grades, can be used to take pre and post tests and can be used for any type of custom survey imaginable.   

SETUP SURVEY
From the Main Menu, click on Surveys / Outcome Measurement 

GENERAL tab - Click the Add New Survey button on the bottom left and type School Grades 2008-2009

  1. OPTIONALLY fill in a Comment

  2. OPTIONALLY fill in Target Number of people in survey

  3. OPTIONALLY fill in Instructions for Survey Sheets

SAMPLING TIMES tab - Click on the Add Sampling Time button on the bottom left

  1. Type in a date

  2. OPTIONALLY fill in a time

  3. OPTIONALLY fill in a comment

  4. Click Add

  5. Repeat for each Sampling Time

QUESTIONS tab - Click on the Add Question button on the bottom left

  1. OPTIONALLY fill in a Question Number

  2. Type in a Question – NOTE: Questions can be anything
    Examples:
    Reading, Writing, Math, Science, etc
    Do you smoke cigarettes, How frequently, Do you drink, How frequently, etc

  3. OPTIONALLY fill in Question Header – this is used as a column header in the Batch Entry Screen

  4. Choose from the allowed answers from the dropdown on the screen for each question

  5. NOTE: new answer sets are created in the View Grade Scales screen

  6. Click Save This Question, click OK at Question Added screen

  7. Repeat for each Question

MEMBERS tab - Click on the Members tab and click on Add Members

  1. Click on the Universal Selection Engine to select specific members to be added

  2. Choose Add all Members or

  3. Choose Add x randomly selected members

  4. Click Add 
    NOTE: It is NOT necessary to add all members to the survey since not all will answer
    NOTE: Members can be added to a Survey from the Members form on the Main Menu

 

ENTER DATA

VIEW/ENTER SURVEY DATA click this button on the bottom to review or fill in the members answers

  1. Choose a Survey

  2. Choose a Unit

  3. Choose a Sampling Time

  4. Choose a Member

  5. Fill in the Answers in the Answer column

  6. Repeat 4 and 5 for each member

BATCH SURVEY ENTRY - displays a list of members with a column for each question with the ability to enter the data across a row for each member very quickly 

  1. Choose a Sampling Time

  2. OPTIONALLY choose to Show Unit

  3. OPTIONALLY choose to Show Membership #

  4. Click on each box and fill in the appropriate answer for each member as the data becomes available

  5. Close to go back to the Surveys section
    NOTE: This screen is sizable and can be moved on the screen
    NOTE: You can right click on the Question columns and sort Ascending or Descending
    NOTE: You can use the tab or enter to move from one field to the next
    NOTE: You can Hardcopy the screen – this will print DIRECTLY to your printer
    NOTE: You can Export this screen to Excel

 

REPORTS tab

PRINT SURVEY SHEETS – this will print the questions and answer options 1 page per member so your members can fill in the answers and hand them in for data entry

  1. Choose a Survey Name

  2. Choose a Sampling Time

  3. Choose to Print for:
    a.       All Members in All Units or;
    b.      All Members in a chosen Unit or;
    c.       An Individual Member

  4. Choose from the Label Answer Choices - this will place letters or numbers in front of each answer
    a.       No Label
    b.      Letters
    c.       Numbers

  5. Choose Number the Questions on the Survey based on the Question Number filled in when adding the questions

  6. Choose to Include or Not Include the Members Address on the form

  7. Choose to Print Short Answers Horizontally

SURVEY REPORTS – this option takes you through the Universal Selection Engine and provides numerous report options

SURVEY ANALYSIS – shows the average and percentage for each question for each sampling time – if you choose more than one sampling time there will be an Avg+- and %+- column which is a comparison of the first to the second and so on

  1. Choose Number of Sampling Times to Display

  2. Choose the Survey Name

  3. Choose the Sampling dates

  4. Preview or Print the report
    OPTIONALLY Click on Open USE to select specific members answers to be shown on the report\
    Click on Close USE to go back to all members who have answered the survey
    NOTE: The Average is the total of the answer scores divided by the number of members who answered
    NOTE: The Percentage is also the total of the answer scores divided by the number of members who answered

QUESTION ANALYSIS – shows an answer breakdown and a pie chart

  1. Choose a Survey Name

  2. Choose a Question

  3. Choose a Sampling Date/Time

  4. OPTIONALLY choose to Ignore ‘No Answer’ or blanks

  5. OPTIONALLY choose to Preview/Print All Questions in this Sampling Time

  6. OPTIONALLY Click on Open USE to select specific members answers to be shown on the report

  7. Click on Close USE to go back to all members who have answered the survey

SURVEY GRAPHS – allows you to create your own charts and graphs

  1. Choose a Survey    

  2. Select a New PAGE for each (Entire Survey is most common)

  3. Select what each COLUMN will show (Question is most common)
    a.       Select All or some of the options in the box

  4. Select what each ROW will show (Time is most common)
    a.       Select All or some of the options in the box

  5. Turn on the Show on Graph for 2 of the options at the bottom

  6. Click the Graph button

  7. You can double click anywhere on the graph and adjust anything on the graph such as color, font size, font color, type of graph and more
